Fertility-Sparing TRT Best Seller

Clomid & Enclomiphene

Clomid (clomiphene citrate) and its purer isomer enclomiphene are oral selective estrogen receptor modulators (SERMs) that prompt your body to make more of its own testosterone. They are the go-to option for younger men, men trying to conceive, and men who want the benefits of TRT without external testosterone.

How it works

The mechanism

These medications block estrogen receptors in the hypothalamus. Your brain reads this as 'low estrogen' and ramps up luteinizing hormone (LH) and follicle stimulating hormone (FSH) — the upstream signals that tell your testes to produce testosterone and sperm. Total testosterone typically rises 1.5–2x over 4–8 weeks while fertility is preserved or even improved.

Who it's for

Best candidates

  • Men with secondary hypogonadism (low T from a sluggish HPG axis, not failed testes)
  • Men actively trying to conceive or planning fertility soon
  • Men in their 20s–40s who want to avoid lifelong injectable TRT
  • Patients restarting natural production after coming off cycles

Protocol

Typical dosing is 12.5–25 mg of enclomiphene daily, or 25 mg Clomid every other day. Bloodwork at baseline, 6 weeks, and quarterly. Many men cycle on/off rather than running it indefinitely.

Timeline

Testosterone usually doubles within 4–6 weeks. Symptom improvement (energy, libido, mood) tracks the lab response and is usually clear by week 6–8.

What to expect

Side effects & considerations

Mostly well tolerated. Some men report mild mood changes, visual disturbances (rare), or elevated estradiol. Enclomiphene generally has a cleaner side-effect profile than older Clomid.
